The management of the patent ductus arteriosus (PDA) is straightforward from a surgical technique standpoint but complex from a medical decision standpoint. The neonatology literature has moved from promoting early, aggressive medical and surgical closure of any PDA in infants to a more watchful approach - reserving mechanical closure to cases of medical failure and hemodynamic significance. Even that definition is one on which it is difficult to find consensus. The ongoing refinement of endovascular techniques for PDA closure will also likely change the face of surgical management in the coming decade.
